Mastalgia (mamalgia, mastodynia) is characterized by pain and tension in the breasts and can be classified as cyclical and non-cyclical. Cyclic mastalgia: this type of breast pain is associated with the menstrual cycle – it occurs in women between the ages of 20 and 50. Cyclic mastalgia is almost always bilateral, diffuse and correlated with the luteal phase of the menstrual cycle. The pain occurs due to natural hormonal changes, about a week before menstruation. Initially a weaker pain occurs in the upper outer parts of the chest that disappears spontaneously when menstruation occurs. It does not require further examination and is treated symptomatically (NSAIDs orally and locally).
Non-cyclical mastalgia: This type of breast pain is not related to the menstrual cycle and is more common in women between 40 and 50 years of age. Non-cyclical mastalgia is most often focal – it affects only one breast, usually in one smaller portion of the breast, but it can also spread. Pain can be continuous or sporadic. Non-cyclical mastalgia can be associated with the use of certain drugs, infection, inflammation, the presence of cysts or fibroadenomas, costochondritis, the presence of scar tissue (post-operative), larger breasts. Non-cyclical pain can also occur due to smoking, caffeine consumption, physical or emotional stress, breastfeeding, lack of essential fatty acids (such as linolenic acid) or iodine deficiency.
In many cases, there is no obvious cause of breast pain, and pain is usually not a symptom of breast cancer.
Patients most often describe breast pain as a heavy, dull pain, but also as a pain similar to a stabbing or as a burning sensation.
